*This document is a legal
and binding contract between Melissa O’Brien here by referred to as the Seller
and _____________________________ hereby referred to as the Buyer(s).
*This puppy is sold with
limited registration, therefore the Buyer(s) has no breeding rights.
The Buyer(s) have to send Seller a statement from the Buyer(s)
veterinarian within 9 months of receiving puppy, stating that the above puppy
has been spayed or neutered. Failure
to do this will make this contract null and void.
*Buyer(s) agrees to pay the
price named above in full to the Seller before the above named puppy reaches 7
weeks of age.
*Payments or deposits from
the Buyer(s) are non-refundable once they have been received by the Seller.
*If the Buyer(s) wants the
puppy named above to be shipped, the shipping cost (which will include crate
cost) stated above must be paid in full to the Seller by the time the puppy is 7
weeks old.
* The puppy will
not be shipped or picked up until the shipping
cost and/or puppy price is paid in full.
*Puppies will not be shipped or picked up until they are 8 weeks old.
*The above named puppy will
not be done with vaccinations or de-worming when the Buyer(s) receives it.
The Buyer(s) will need to take the puppy to their veterinarian within 3
days of receiving puppy so that he or she can recommend future vaccinations and
routine care. Failure to do so will make this contract null and void.
*Puppies are guaranteed for two years from date of birth against life threatening congenital defects. If a life threatening congenital defect is found within the two years, the Buyer(s) must send the Seller a statement from the Buyer(s) veterinarian (with his or her address and phone number on it) stating exactly what is wrong with the puppy. Failure to do this will make this contract null and void. If it is an actual life threatening congenital defect the Seller would then give the Buyer(s) another puppy of the same breed, sex, and color as soon as one becomes available. Seller will not give a cash refund for any reason. The Buyer(s) would be responsible for any and all shipping charges for new puppy.
*All puppies will come with a health certificate from Seller's veterinarian.
*All puppy registration
applications are AKC and come with Glendale’s as the beginning of the puppy’s
name.
*If Buyer(s) are unable to keep or take care of puppy for any reason, Seller will take puppy back. Buyer(s) will be responsible for all shipping charges occurred.

THE SELLER’S SALES CONTRACT AND HEALTH GUARANTEE.
